Notes
- At the time of Mack's birth and death soon after, the family was living in Earl, Oklahoma Territory. One historical source indicates this was in J County, which later became Johnston County. In the Oklahoma Territory many if not all the counties were named simply with letters of the alphabet. Oklahoma Territory and Indian Territory were later merged into the new State of Oklahoma in 1907.

However, modern Johnston County was created at the establishment of the State of Oklahoma, in what was then Chickasaw Naiton, which did not become a part of Oklahoma until the Indian Territory was merged with Oklahoma Territory in September 1907. So it appears Earl was actually in Chickasaw Nation at that time. This puzzling note about J County and Johnston County has been removed now in the revised and relocated OKGenWeb site.
"Johnston County was created, at statehood, in 1907 and wass named for Douglas H. Johnston, governor of the Chickasaw Nation. It is located in southern Oklahoma and covers 645 square miles. The county seat is Tishomingo. The Chickasaw capitol building built in 1896 became the county courthouse."
